When the inlet side pressure is lower than the preset pressure, the diaphragm is closed with force of the spring. When the inlet side pressure is higher than the preset pressure, the diaphragm is jacked up, the medium go though the valve. Safety valve can also be worked as back pressure valve, only change the inlet and outlet direction.
